Office Hours

 
Our office is open from 9:00am - 4:00pm Mondays - Fridays.
 
After hours, Dr. Levine or one of her associates will be available for emergencies. Instructions for contacting Dr. Levine or her on-call coverage will be available by calling our main number 358-3582. You can also leave a message for less urgent matters.
In the unlikely event that our phone system malfunctions or you do not receive a timely response, please go directly to the nearest emergency room.
 

Appointments

 
Patients are seen in the office by appointment. For your initial visit, please arrive fifteen minutes early to complete the necessary paperwork, Alternatively, we can mail you the forms in advance or you can download them from this site. We strive to run an on time office. We appreciate that you are busy and your time is valuable. Your cooperation is necessary to keep us on schedule. If you are unable to keep an appointment, please telephone the office at least one day in advance to reschedule. We will make every effort to notify you in the event that we are delayed by an emergency.
 
Patients need to be aware that they will be charged for a missed appointment if giving less than 24 hours notice.
 

Prescription Refills

 
Dr. Levine carefully chooses both the amount and type of medication to prescribe for you, during your office visit. The last authorized refill should serve as a reminder to you of the appropriate time for follow-up. If you do need a refill to tide you over, please have your pharmacy fax a request to us at (408) 358-3468
 

Telephone Consults

 
Dr. Levine prefers to meet with her patients face to face, but knowing how hectic and busy your Silicon Valley life can get, phone consults are also available by appointment.
 

Patient Relations

 
Matters of a sensitive nature are apt to be discussed in the office; all communications are strictly confidential. The only exception is when required by law.
 
If you are under the care of another physician for an active problem, please request that your records be sent before your first appointment. The law requires that such a request be made in writing. Also, please bring the names and dosages of all medicines that you are taking, especially if you need a refill. The receptionist may request more information over the telephone, for appointments other than annual exams. This additional information allows us to schedule sufficient time for your visit, particularly if the issues are complex.

We contract with many insurance plans and accept cash, checks, Visa and Master Card.
